Please fill out this form as completely as possible and submit NO LATER THAN November 8, 2009.  You will then be contacted to schedule a personal interview with Pastor Laurie Hurd.  Interviews will be conducted at the church office on November 12 & 13, and again on November 17 & 18.

Please disregard any difference in color between fill-in boxes.  There is a glitch in the web software that cannot be resolved.  Fill out the application in its entirety.
